A fascinating new preprint w/one very unexpected finding suggests, I believe, that a large proportion of Long Covid may be due to chronic infection in a particular bodily niche, which could be crucial for finding effective LC treatments. It requires some explaining. 🧵 1/33

I wonder how this applies to cancers in general outside of breast cancer. Asking because 3 of my youngest's coworkers (20's to 30s in age) are currently battling cancers. She believes all 3 have lung cancer.

Neuro-PASC emerges from a multi-hit cascade: genetic and environmental vulnerabilities, acute SARS-CoV-2 infection, and maladaptive immune responses. Targeted therapies must address blood-brain barrier repair and chronic neuroinflammation. davidlingenfelter.substack.com/p/the-conver...

________________________________________ 🧠 New Investigative Report on Neuro-PASC Pathogenesis by David Lingenfelter, PhD In a comprehensive synthesis now available on Substack, Dr. David Lingenfelter presents a multi-hit model explaining the neurological sequelae of Long COVID (Neuro-PASC). The report integrates genetic susceptibility, environmental exposures, and immune dysregulation to reveal how SARS-CoV-2 disrupts the blood-brain barrier and induces chronic neuroinflammation. Key findings include: • Identification of FOXP4 and HLA loci as genetic risk factors for hypothalamic dysfunction and autoimmunity • Evidence of spike protein–mediated BBB compromise and variant-specific endothelial damage • Persistent T-cell exhaustion and autoantibody production as drivers of chronic symptoms • Implications for biomarker development and precision therapeutics targeting glial activation and cytokine signaling This work reframes Neuro-PASC as a convergence syndrome—where host vulnerabilities and viral triggers intersect to produce lasting neurological dysfunction. It offers a roadmap for future research, clinical stratification, and targeted intervention. 📚 Recommended for clinicians, neuroscientists, immunologists, and public health researchers tracking post-viral syndromes. ________________________________________ Here's a structured summary of the key insights from my Substack report titled “The Convergence of Host Genetics and Environment in Post-COVID Neurological Sequelae”: ________________________________________ 🧠 Central Thesis Neuro-PASC (neurological post-acute sequelae of COVID-19) arises from a multi-hit pathogenesis model: • Hit 1: Baseline vulnerability (genetic predisposition, comorbidities, environmental exposures) • Hit 2: Acute SARS-CoV-2 infection • Hit 3: Maladaptive host response—marked by blood-brain barrier (BBB) disruption and immune dysregulation ________________________________________ 🧬 Genetic Susceptibility • FOXP4 locus: Linked to pulmonary function and hypothalamic expression; associated with fatigue, dysautonomia, and sleep disturbance • HLA-DQA1–DQB1: Implicated in autoimmunity and maladaptive immune responses • Cytokine genes (IL-6, IL-10, TNF-α): Influence inflammatory tone and chronic immune activation • Genetic overlap: Shared risk architecture with ME/CFS, fibromyalgia, and depression ________________________________________ 🌍 Environmental & Epidemiological Risk Factors • Demographics: Female sex, young to middle age, non-white race, and Hispanic/Latino ethnicity • Comorbidities: Diabetes, cardiovascular disease, obesity, and pre-existing neurological or psychiatric conditions • Environmental exposures: Air pollution, poverty, chronic stress—all prime inflammatory and vascular systems ________________________________________ 🧪 Pathophysiological Mechanisms 1. Blood-Brain Barrier Disruption • Direct damage from spike protein and viral variants • Indirect damage via cytokine storm, endothelial dysfunction, hypoxia • Genetic vulnerability in BBB integrity genes (e.g., CSMD1, PTPRD) 2. Immune Dysregulation • Chronic neuroinflammation driven by persistent cytokine signaling • T-cell exhaustion and senescence impair viral clearance and sustain inflammation • Autoimmunity via molecular mimicry and epitope spreading __________________________…

5. Removing their mask in a public restroom because "nobody else was in there". Bad news--airborne means that virus will linger in the air for a while & public restrooms often aren't well ventilated. They're small, cramped & damp--environments that allow an airborne virus to linger in the air.

Good conclusions, pretty sure my government will do nothing with it whatsoever. (In my country, they don't even bother to count the number of people affected with #longcovid).

I wish people would stop blaming COVID mitigation policies for the acceleration of right wing extremism, and instead acknowledge the fact that we endured a mass casualty and mass disabling event that was mismanaged by narcissistic authoritarians who shifted the blame to marginalized communities

As one of Jonathan's ~250 subscribers, I can confirm that it was a tiny & unnoticed channel where he was simply preserving them speaking their own beliefs freely wo taint of opinion. It was a historical record without hype. All I can figure is that Prasad was searching for himself online again.

Please do. I haven't been able to read a book in ages due to my vision but you know what? I should order a copy, too, to fight this revisionism. The more of them that are out there, the more likely it will be remembered what actually happened.

I'm so glad to see this story picked up by the Guardian. Dr. Jonathan Howard is one of my mutuals here on bsky & was talking about it today. I was one of his subscribers. Most of his videos were literally 3rd party interviews w Bhattacharya, Prasad, & the rest. Clips w little or no commentary.

Joined this group as an escape. Didn't expect to be confronted with that. I feel so terrible for him. I don't know if he got sick & had this happen to him or if his mind broke from the strain of everything. I just don't know but it was honestly heartbreaking. 😭

As I explained it, he listened sincerely, nodding, and said, "maybe...maybe...", contemplating the possibility. He was calm the whole time, accepting of his new reality. In his mind, he was adjusting to the framework of being a quantum jumper. Instead, he was adjusting to probable psychosis.

I explained to him about how, for some people, their personalities might change after COVID infection. He asked me how often it happens & I told him that I didn't know but I'd seen it several times with people I knew. I offered that maybe some of the people who'd changed might just have gotten sick.

He told me that he had finally figured out what was happening to him. He was quantum jumping. Every time that steel truck appeared, he'd quantum jump into another universe where everyone would change. I explained to him that some of the people changing & disappearing might be more explainable. 🤞

He was totally calm the entire time even though he also seemed distressed & contemplative. I asked him when this started & he said 2 years ago. I said that he was taking all of it well & he said that last year, he was terrified & scared but now, he was dealing with it better. 😭

He told me that he'd been seeing that happen or people he had known for a long time just disappear as if they had never existed at all. He said that he'd finally figured out the pattern & it was when he saw a specific steel truck and a few specific cars, then the timeline would change. 😰

I told him about my rheumatologist as an example & he agreed that was weird. But, he said, he'd seen them change even bigger than that. Like physically change. One of his friends that he'd known since they were in the 1st grade had changed to now be 20 years old again, dropping away decades. 👀

I said I had been, too, but what I thought he was going to say was not even what I ever imagined. He told me how the people he knew were behaving differently now, like they had completely different personalities. Chagrined, I told him that I'd seen that happen, too.

Someone from the little medieval fight in the park gang was injured this weekend so I moved it to provide some first aid (knee injury). I ended up just kind of hanging out w him afterwards to make sure he was fine & after about 10 minutes, he told me that he'd been seeing strange things happening.

I think part of the issue is people go "but no-one would accuse my daughter of being trans!". You need to really hammer home - "Remember that creep teacher we had? Think what he would get away with if this passed and he has an excuse." Because everyone had a dodgy teacher at some point.
