That's an IV site that infiltrated.
It does look like someone blew the vein. Can also happen when the vein isn't in great shape — e.g. if the patient is older. Definitely looks IV-related. Since it's on the backs of both hands, even more likely. CHF? DVT? ESRD? Inflammation?
Or neurosyphilis? Would explain the leg drop.
PS to that reply: Parkinson's is more likely here, given some other symptoms: having to drink water using both hands; shuffling gait; balance issues; disorientation; frequently missed words. (Hey, I worry about this stuff myself & my PCP always laughs at me when I bring it up.)

I wouldn't suspect a long-term syphilitic infection simply because diagnosis is fairly straightforward — and therefore penicillin treatment would have been initiated long ago with full recovery by now. Of course, that diagnosis is not impossible & shouldn't be overlooked.

Well, we know he is getting IV Lasix on a frequent basis. I'm surprised they don't just leave the hep-lock in. That also explains the need for a catheter.

The extent of peripheral edema (i.e. his "cankles") would recommend against increasing fluids via IV. Could be massive prednisone dosage to combat inflammation — would help explain the mood swings, rages, etc. I wouldn't be shocked if they're also giving meth, ketamine, and more.
I’m sure he’s dictating his treatment preferences. I’m also sure there’s something like at least ketamine involved. No way to know for sure but unlikely and unwise treatments often demand unwise solutions.
My bet is meth+ketamine+prednisone all via IV. Explains the rages, mood swings, swollen face. Maybe a chaser of something to mitigate mental decline, possibly a painkiller not and then (explains the slurred speech).

Might be too much fat at the elbow for them to get a good shot at a vein. Other sites might have poor access, bad veins, etc. Then, too, maybe they HAVE been using other locations, but those sites are over-used & now they have to go for the hands.
Ask a phlebotomist. Gotta find the good veins.
