A voter address confirmation postcard from the Seminole County Supervisor of Elections (Florida) is shown, addressed to “George Washington, 1500 E Airport Blvd, Sanford FL 32773.” The card is a bilingual English/Spanish form and consists of two parts: • Left Side (Form to Fill Out): • Pre-filled voter name and address. • Boxes labeled “My Current Residential Address,” “Mailing Address,” and “Name Change,” all of which are blank and can be filled out by the voter. • A bold box stating “VOTER MUST SIGN” (with Spanish translation). • Signature line labeled “SIGN HERE” and “DATE.” • Right Side (Mailing Panel): • Printed return address: Vote Seminole, P.O. Box 1479, Sanford, FL 32772-1479. • A section marked “Return Service Requested.” • Instructions titled “Address Confirmation Request” stating this is to confirm or update a voter’s name/address on file.
