Huh. Well, I do recall getting a prescription from our Dr! Maybe that was just his thing.

It might depend on which vaccine is being administered and if it will be administered off label. WA is very proactive regarding vaccine administration. Gardasil for HPV can be given to minors 14 & up without parental consent because the law allows them to consent to care for STD’s.
