The Loop of Henle. Trump's is failing. Meaning his kidneys are failing. Meaning his current treatment of loop diuretics for congestive heart failure, will fail. (click)

The principal function of the loop of Henle is in the recovery of water and sodium chloride from urine. Which means it separates waste products from blood. Trump's isn't, leading to fluid retention in ankles. The bruising on the hands is from loop diuretics, the treatment for congestive heart failure. Remember, Trump said he had Chronic Venous Insufficiency. CVI ISN'T treated with loop diuretics. CHF is. But, it relies on healthy kidney function, and Trump's kidneys are failing too.
