Happy to be corrected on this but I don’t think it’s even really speculating to say that if this isn’t from a regular intravenous infusion for an incurable disease (or blood thinner use for a different kind of incurable disease) then he’s in worse shape than people are thinking

I have a friend who takes Asprin to help keep her blood thin because of clots. She said she only has to bump her hand slightly and she bruises. Maybe fvck nuts trump is on Aspirin or some type of blood thinners.

Based on other photos I've seen, the bruising is inconsistent with where they would be sticking him for an IV infusion. If he needed them frequently, and his veins are that bad, they'd definitely want a PICC. Whatever he has, I hope it's painful.

What are the odds the infusions are entirely benign? He's old and in bad shape and is constantly on the move. It could just be electrolytes and fluids to keep him active, or some other mix or random things they give people at IV bars.

Someone more versed in when fluids are indicated/contraindicated might chime in here but listen, if my grandmother's ankles looked like that there's no way she's remembering her lasix and she's FULL of sodium, like we got *problems*. If he's getting IVs they're not addressing it or he's in BAD shape

some people will hit veins they figure will work. When his hand has been down like that, I can see that one vein that someone could poke, but that’s not always a guarantee you’ll get blood🤷🏻‍♀️ you should rotate sites but I’m aware not everyone does that for phlebotomy. Not best practice

I’m pretty convinced his issues are primarily cardiac. He’s almost certainly on anticoagulation. Suspect CHF, maybe advanced kidney disease as well. Definitely mild to moderate dementia, perhaps vascular. All speculation but it fits available info.

WAG since I know someone with Hemochromatosis (build up of iron). The treatment requires routine blood filtering to remove excess iron. Diet can lower frequency. It’s most common in Northern Europeans.

I get regular infusions for an autoimmune disease and I almost always get them in my hands. Easy veins there, allows patients a little more movement while getting the infusion. I think it’s fairly typical (though I don’t think his bruising is caused by IVs, I’d guess)

I have a bleeding disorder and I get infusions nearly every week and I would never have them done in my hands. At 75 my hands bruised to easily and I have great veins in my forearms that don't hurt like they do in my hands

I've seen it 100s of times. Bruising on hands of elderly patients on Aspirin, Plavix, Coumadin. They have thin skin anyway. Hands are not great for regular IV infusion. You'd want vein larger, like antecubital. His hand bruising isnt significant (sorry to say.) Watch for a fall w head injury soon.

Not a physician, but from caregiving & volunteer experience with older individuals, that kind of hand bruising to me is just indicative of 'man of a certain vintage on blood thinners who smacked their aged, thin-skinned hand against a door frame/golf cart/etc'

This is true - I think the only two meds I remember them ever listing were aspirin and a statin(?). With the venous insufficiency, I'd be surprised if he's not also on a NOAC and that wouldn't be news, but that really should be mentioned during his *glowing* medical reviews

My first thought was "coumadin bruise" which we see all the time. And if that's the case extremely likely to be because of a-fib too. Neither of which would be unexpected on that age group but they would never admit it.
