Maui Mallard in Cold Shadow The game over screen freaked me out as a kid. There's something very unsettling about seeing a guy who looked like Donald Duck (and I think he was originally marketed as Donald Duck acting as Maui Mallard)

get pulled into hell with the ground opening up under his feet and a bunch of green hands do the job. And, on top of it all, the game was HARD, so I saw that game over screen A LOT.
