As someone from rural Ohio (the Appalachian part), I want to wring my hands and go, "sucks to suck. Go and do your rebel yell at a funeral." But even in Trump country, ~20-30% voted for Harris. There are people who voted to keep Medicaid/Medicare for themselves AND others.
So only the rich will be able to afford nursing home care and hospital care.

The Medicaid reimbursement rates are already too low to provide decent care; any lower and it becomes unworkable. ⬆️ unemployment in rural areas, ⬇️ tax revenue, ⬆️ ER visits, ⬇️ hospital reimbursement, see where this is going?

Saw this happen as a one-off as a unit RN. Dispute between medicaid and patient family over ownership of a home. Patient couldn't qualify but needed a chronic vent facility. Tied up a hospital critical care bed for months while it got sorted out in the courts. Can't imagine it at scale.
Case managers and social workers already have an impossible job.

It depends on their income. If they do not wander or have behavioral issues, they can go to a SNF. If they need more care but can’t afford memory care, maybe a board and care which are wonderful, but still 3-4K a month
My aunt had dementia with behavior/wandering issues. Her personal assets covered several years of a secure nursing facility and Medicaid picked up when her cash ran out. I don’t know what we would have done without Medicaid- we couldn’t afford to pay for her care, and had no way to keep her at home.

Alot of people have no idea how to handle dementia or other memory care. Many don't know the basics of just helping an elderly person that can't clean themselves. People will suffer and xie.
A lot of people think that dementia is just being really forgetful. When they aren’t treating it as a punchline. As someone whose father died of Lewy Body, it ain’t that.
Yup. I work in assisted living so we deal with it firsthand. It's way more complicated than that.

Healthcare in the US has always been problematic, this just means now when people/employers pay for their health insurance they still won't have access to medical care. I guess it isn't much different than the repeated unnecessary denials they faced before though, right? It just skips some steps.

Also, the "defer modernization efforts" is going to sting. Those probably refer to things like detecting patients falling in their room with AI, or additional cooling in the rooms to deal with climate change. Deferring those can and will kill a lot of grandparents...

I think they are screwed too, just the reckoning will be a lot subtler and more delayed. The king of the wasteland still lives in a wasteland. Like being able to afford the best medical care means nothing if you die in transit because you are five hours away from a hospital.

Living in rural America is already a huge challenge for medical care. Doctors don’t want to live and raise families here for the most part. 60-90 minute drive to large hospital if ours closes. Not to mention the jobs lost. But, I guess we’ll “get over it”

Honestly the problem is cyclical; they feel condescended upon by wonky urban elites, which makes them lash out in self destructive rage, which makes those wonky urban elites point out how stupidly self destructive their behavior is, which makes them lash out with even *more* rage... and so on.
A comparison i make is to when one of your friends (or yourself!) Is acting childish, as we all do sometimes, and you tell them so -- and often, that just makes them angrier. Their bitterness grows, and so you point out that theyre having a temper tantrum. Which... only makes things worse.

Everyone seems focused on rural hospitals but urban ones serving all comers will be in just as much trouble, won't they? Plus cuts to residency programs, visas for workers, and fewer people with any insurance due to ACA changes -- the whole system is at high risk of collapse.
