I have received letters like that before from SS. I have been on SSDI for 20 years, at about 18 years in, they stopped sending them to me. I have received them asking about my health via questionnaire. I never had to go to a Dr and get her to fill anything out. I don't think it's anything sinister.

It looks standard. But I never received the "first" letter they said they sent. I can't remember if they sent me a similar form a couple of years ago. I had to get it notarized, too. I might just be paranoid.
