If insurance prices increase with salary, then why not simplify everything, cut out the middle man (the insurance company, whether it's profit based, or non-profit) & fund the health system through general taxation?

Interesting, but not convincing. I have no children*, so why do I have to pay taxes for schools; I'm a pacifist*, and it's wrong to make me pay taxes to fund the army;... The govt we elect should determine which health aspects should be covered, & raise the necessary taxes. *not necessarily true
Under taxation rules no-one gets to decide who or what is worthy of treatment. If we chose not to pay for eg dental treatment, later costs from rotten teeth would require more funding. No opt-outs. No lucky/smug healthy living discounts.
