The judge needs to approve of an insanity plea. He would need to be examined by a court approved psychiatrist to determine if his actions were committed due to insanity or if he understood right from wrong. Insanity is notoriously difficult to plead in court.

Like do people even understand where health insurance profit comes from? They have a profit margin around 5%. They are required by law to spend at least 85 cents on claims for every dollar of premium paid and about 10 cents is needed for expenses. They make profits through volume, not denials
