I think his guts are rotten and he's at risk for sepsis, as well as coronary/vascular issues.

That's likely. Hell, maybe the dementia has reached the terminal stage too, and I wouldn't be surprised if he's suffering from bowel cancer, given his diet. There are definitely grounds for optimism. 😊
