The plea deal was for only two offences and naturally couldn't have included anything that happened years later. Under US law, the double jeopardy protection doesn't apply across federal and state jurisdictions. A person can be convicted for the same act separately under state and federal law.

Florida started the case against Epstein, but state law enforcement was railroaded by Acosta and the federal plea agreement. They could have pursued state charges as well, though, but I assume they were told to leave it alone. Eventually New York did pursue and viola, Epstein is dead.

To my knowledge, the FBI started investigating him in 2019, some time before he was arrested. Between 2011 and 2018, I'm not aware of any authority investigating his continued CSA activities and trafficking. Whether it should've been done by the FBI, FL, or NY, that I don't know.
