My father passed away 8 years ago - he had this look. Sudden weight loss, skin hanging, a confused/lost look, sunken/puffy eyes, stopped dressing or grooming in his normal manner - it was all dementia and heart failure.

I have seen this with my Dad too. It started 6 months before he passed away.
