Ajja is increasingly upset because a plague ship has arrived at the harbor and infected a huge number of harbor workers, and Ineal is one of a very small number of Sickly who are producing a viable cure. the blood draws are growing increasingly dangerous, and Ineal is incapable of any real consent.
In a setting with the worldbuilding you've described, being concerned about consent sounds like the abnormal view...
so, the thing is that it's not... anymore. but everyone is now living in the ruins of a civilization which absolutely didn't care about those things, and so much depends on factors which humanity no longer has any control over. there isn't really a pharmaceutical industry anymore. there are Sickly.
like, Tomas agrees that they shouldn't have engineered his people to die in their 40s of cancer and consigned them to a lifetime of tiptoing around their killer instincts, but they did, and to remediate the harm which was done to his ancestors would be genocide.
how do we take the fact that Ineal can't understand her own mortality, and genuinely believes she'll survive and everything will be fine? someone has to fear on her behalf, because she can't do it herself -- but other people who _do_ care about their lives will die if no one risks her.

yeah, i'm putting together tabletop game stuff to run tomorrow night. the players are going to be investigating a plague ship which ran aground in a major city, and the major ethical conflict they're going to run into is around these three characters.
