There are a few approved conditions that seem more nebulous - do I have to be officially diagnosed by my doctor? If I go get the vaccine anyway, is my insurance going to come chase after me to pay later? Are we going to lose access to the flu vax also?

You can request and receive a vaccine at any pharmacy. They can check to see if your insurance covers it for you as well. The underlying conditions cover >6 mo across the entire country and depression and lack of physical activity are included. Most people know this and are unconcerned.

i see that you’re replying to everyone with basically the same thing, so probably a bot, but I guess i’ll bite. No, there is no guarantee that insurance won’t deny coverage for a vaccine you’ve (general you) already gotten, they do this already. And only people not paying attention are unconcerned.
