It's clearly doctored. The font doesn't even match on the captions.

Annoyingly, all of the relevant images on this page are broken for me, so I can't see if it's actually the same image or just a similar one.

It was the same picture with the same fonts. It is interesting that a different font was used on the second picture. My MAD magazine days ended in the 70s so I am not sure what happened to their editing. Snopes confirmed the picture in MAD magazine in 1992.
