Why is he wearing that hat covers most of his head & not his usual MAGA ball cap? It's hiding something. Maybe surgical gauze from a craniotomy.

It's one of his white "USA" ball caps. It looks weird because it blends into the background in shadow, but you can see the shape of it if you look closely.
