If a health provider or specialist leaves your health Ins network, health files don't automatically go with you & sometimes new providers are not up to date. Emphasizing the 'P' in HIPAA would be nice - AT ANY TIME OTHER THAN THIS. Now information is being AI'ed, used to surveil ppl & weaponized.

They are probably billing insurers, so yeah they can be considered a covered entity. But Apple and other apps/devices that you store this health information and data with, are not bound by HIPAA regulations. Just general FTC regulations against unfair/deceptive practices (vague) and state laws.

It’s about whether the institution at any point deals with making claims to insurance or they process those records (ie clearinghouses and yes EMR repositories). Not about how you interact with them. Apple does not make insurance claims as their business so no HIPAA. State privacy laws may apply tho
