Asserting Dr - Patient privilege is not the same as "refusing to cooperate." It's wildly inappropriate to ask a doctor to answer questions that violate privilege in the first place.

Not really, but asserting a/c privilege is also not "refusing to cooperate." It protects the broader legal system by ensuring all participants can be truthful with their lawyers so they can get appropriate legal advice.
