Matthews, who lives in rural Missouri, relies on Medicaid for health care — which she only gained after the state of Missouri expanded access in 2020. Now, Republicans’ “One Big Beautiful Bill” threatens that coverage with proposed work requirements and cuts to rural Medicaid funding.
The bill proposes work requirements for “able-bodied” adults ages 19-64 with exceptions — including for those with children under 14, like Matthews: “How is that fair to just take it away from somebody that does have to worry about it because of finances or because of a job?”

The new proposal, which limits federal Medicaid reimbursements to states, could cost rural hospitals billions of funding by capping provider fees. Health care providers, hospital associations and even some GOP lawmakers, including Sen. Josh Hawley, warn the measure would “defund” rural hospitals.
Rothermich said the hospital has made tough cutbacks, including freezing salaries and closing a home health agency that rehab services. “There are fewer and fewer people to take care of it and fewer and fewer resources to try and do what we need to do,” he said.
Karen White, administrator of Missouri Highlands Health Care in southern Missouri, said the proposed changes could force them to prioritize patient care. With three hospitals lost in the region over the past decade, she said, Missouri Highlands has become the area’s “only form of health care.”
The American Hospital Association estimates rural hospitals could lose $50 billion over the next decade under the GOP’s provider tax plan, with overall hospital losses reaching $300 billion, according to the Urban Institute. nbcnews.to/40rdTMA
