Well of course! When you get new machines the planar orientator has to be set to equator (or wherever you are) settings. They tested it at the machine factory in Europe or wherever, everything worked fine there and they didn’t bother to run a test after. Rookie mistake if you ask me.

...I would posit a guess that the error occurred at the moment the stack of tissues was shoved into the box. As you can see in the second video above at around the 1 minute mark, the tissue stacks arrive at this stage sometimes not fully squared. ...
...as the Big Smasher Thing comes down to press them smaller to fit the box, a slight maladjustment could, theoretically, cause the whole stack to shift sideways and then be shoved into the waiting box, where shipping and handling will cause the tissues to settle in more fully sideways.
