I’m guessing that Trump has been suffering from kidney failure since his first term. The bruising on his left arm is consistent with post-dialysis issues. His edema and skin problems are a result of excess fluid collecting, due to his kidney insufficiency.

The fistula would be further up his arm, but using dialysis in the White House might be done by less experienced medical staff resulting in hematomas down his arm. The edema is pretty serious and kidney failure would be advanced. The worse scenario is congestive heart failure.
