One of the great outcomes of attribute based damage is players DEEPLY CARE about their wounds. It isn't like needing to top up that HP tank. Oh no- they desperately want medical attention, bandages, salves and a warm tavern after getting fucked up in the wilds. They WANT their stats back.
Or have enemies leave you for dead and break off the combat if I can- pending things like enemies doing crits to finish you. This makes for more dramatic situations where the party get their ass handed to them, are humilated or robbed and then can later get revenge.
Now the "death spiral" issue. Note that the stat you degrade doesn't have to be the one that affects the current fight. This means you don't get the "death spiral" of diminishing attributes as much as those that directly affect the fight until a ways into the fight. I always allow players to flee
Taking CON damage or CHA damage doesn't effect the current fight. So you don't get the feedback loop until you are forced to pick strength or dex. See?
Yes, if the damage was purely physical. Also the answer to death spirals is allowing the players to flee or surrender in a much more forgiving manner, and also having enemies disengage and leave you for dead. This makes for great drama where players don't get TPKed, but get revenge later.
Like hit points, any attribute reduced to zero puts you into a critical save state. Because you have to pick one stat to eat each blow, you can weather a wide weak beating up, but only a few large totals of damage at once. Give it go
Opting for wisdom or intelligence is essentially a concussion or black eye.
You can have the players engaging with the damage by picking which ONE stat they take damage on with each hit. Clegane chose permanent charisma damage, making him physically uncomfortable for npcs to engage with, but also bitter and rude.
Attribute damage has the effect of removing the "nearly dead, but swig a potion and spring back up" you get from the on/off effect of running out of HP. You are being chipped away, both partially and permanently in a range of ways.
This damage can usually be temporary, however having permanent damage on, say, a critical hit or an attack that does more damage than your current stat, gives you old war wounds that affect the base stat.
A neat way of handling damage in your homebrew TTRPGs is to not have hit points at all, but instead deduct from your stats directly. This effectively hampers characters in realistic ways, making them literally limp along, addle their brains, scar them emotionally and physically.

I remember that because I was one of the effects artists stuck in a warehouse in Sydney doing horrendous hours and never seeing my family for 'our industry'.
The fact is we had a golden age when Hollywood was shooting here (like Starwars and Matrix) because of government subsidies and tax cuts, and when that dried up everyone waving Australian industry flags fucked off overseas.
I distinctly remember being courted to do storyboards and concept art for that, then the production office went overseas.
