Agreed. If you're not on it already, may I suggest a round of paxlovid + zinc, high dose Vit C and D and metformin for 4 weeks? Seems to reduce likelihood of long COVID by 40%. Certainly worked for me.

I generally go from 3 straight to D if I don't get it first go. During the initial COVID waves I started routinely using a bougie to improve my FPS rate, and it's just a short step from that to routinely using HAVL + stylet/steerable bougie from there. I teach VADL but use (HA)VL routinely.
Was discussing a case recently where a non-specialist doctor did an RSI in a young female, couldn't see anything with a CMAC-3 and went for a 4 instead of a D-blade. Broke three of her teeth and bloodied up the pharynx - couldn't see a thing and had to go to FONA.

It starts with us in theatres. We prescribe oxycodone to manage pain acutely in the perioperative period, but there is no active weaning/transition strategy on the wards that is coordinated with the depts of surgery or medicine. So JMOs just continue to prescribe the inpatient meds on discharge.
Was having a discussion about this with a colleague yesterday. Sadly I think we're heading for the stage where apart from the true NOVIDs, there will be no 'long COVID'; all this new debility will just be absorbed into the general population; it will become part of being human.

E.g. it took me about 5 years to realise that there's massive variability in how anaesthetists are taught and perform subtenon blocks. This makes interpreting research on the subject quite problematic

I've occasionally been asked in job interviews "what's the best anaesthetic you've ever given?" Each time I fumbled a mediocre answer. If I get asked again, I'll say "I don't know - it hasn't happened yet. But I'm getting ready for it"

One can carve a practice niche for oneself giving excellent GAs and parenteral analgesia to all one's patients without ever having to perform a regional block ever. There is a valuable place for such practitioners. My practice is different.
Actually I think the opposite - I feel my generation had that multimodal approach integrated into our training and now we're losing it. But I certainly agree with the premise.

1. "complex" rather than "complicated" 2. yes one could flip a coin but part of our craft is to assess the complexity. Over time we learn and exercise judgement. Even if it ends up that half the time it's RA and half the time it's GA... the choice between the two is not necessarily a random one.

I'm quite willing to entertain this argument. We all know for example that complex regional block techniques (e.g. catheter infusions/PCRA) require good acute pain service infrastructure/education/follow-up without which one could argue many pts are better off with an IV PCA.
