Agreed. If you're not on it already, may I suggest a round of paxlovid + zinc, high dose Vit C and D and metformin for 4 weeks? Seems to reduce likelihood of long COVID by 40%. Certainly worked for me.

I generally go from 3 straight to D if I don't get it first go. During the initial COVID waves I started routinely using a bougie to improve my FPS rate, and it's just a short step from that to routinely using HAVL + stylet/steerable bougie from there. I teach VADL but use (HA)VL routinely.

Was discussing a case recently where a non-specialist doctor did an RSI in a young female, couldn't see anything with a CMAC-3 and went for a 4 instead of a D-blade. Broke three of her teeth and bloodied up the pharynx - couldn't see a thing and had to go to FONA.

It starts with us in theatres. We prescribe oxycodone to manage pain acutely in the perioperative period, but there is no active weaning/transition strategy on the wards that is coordinated with the depts of surgery or medicine. So JMOs just continue to prescribe the inpatient meds on discharge.

I generally carry a bag of coloured sharpies with me. For TURPs under spinal I quickly draw a beach scene with a water's edge, a beach ball, a starfish or two, a happy shining sun etc. I tape this to the inside of the blood brain barrier so that patients have something to look at.

I'm quite willing to entertain this argument. We all know for example that complex regional block techniques (e.g. catheter infusions/PCRA) require good acute pain service infrastructure/education/follow-up without which one could argue many pts are better off with an IV PCA.
