Autism, childhood neglect, depression, and parentification explain her clumsiness, her mostly flat masking, her unwillingness to be vulnerable, her stubborn independence, her discomfort with people concerned about her, even overwhelm in the dress shop. It's a vastly different read of the character.

Between the AuDHD and ear issues, I spent a lot of time anticipating what people would say in a situation, unconsciously memorizing social scripts and patterns of conversation. So I'd end up answering questions before people finished asking, irritating them whether I was correct or not. Fun, right?
I also need to look at lips or have subtitles. My brain has been good at filling in gaps since childhood. When my age was still in single digits I had a lot of ear infections (after my tonsils came out at 2), and my pediatrician told my mom that I was hearing things as if under water.

When writing your first draft, tie up your internal editor and critic and write. It doesn't matter what those two say, just put whatever is in your head down on paper/doc file for later revision. You can't edit a blank page.

There's no reason to even base a fantasy world around historical designs, but even so, historically, most people bathed and found a way to deal with sewage and waste. I like to think that one of the things that make hobbit holes so cozy is because they had indoor running water and a loo.
Historically, Ancient Minoans had flushing toilets, Romans had indoor plumbing, Mayans had pressurized water for irrigation, potable water, and sewage, to name a few. (Feel free to jump in with other ancient cultures.)
One thing I now ensure incorporate in my fantasy stories is indoor plumbing. My view: It's my fantasy world, it can have showers and flushing toilets if I want it to. Not everyone needs to be stinky and miserable like some stereotyped, ahistorical Medieval English peasant.
