Historically, Ancient Minoans had flushing toilets, Romans had indoor plumbing, Mayans had pressurized water for irrigation, potable water, and sewage, to name a few. (Feel free to jump in with other ancient cultures.)

There's no reason to even base a fantasy world around historical designs, but even so, historically, most people bathed and found a way to deal with sewage and waste. I like to think that one of the things that make hobbit holes so cozy is because they had indoor running water and a loo.
