I was trying to say, and very poorly it seems, that those are criteria which will allow you to get the vaccine. Those and a host of others.

My husband's son has asthma and needs that vaccine. But should he switch jobs and have a new insurance company, they could deny him coverage based on a pre existing condition. Trump's regime wants all of our data. Look what they did with our SSI info.
It’s possible. I just entered as a 35 year old and it accepted that I qualify. Nothing was asked specifically and was not told to bring any sort of documentation. They will have a form to fill out for health history but if his son has visited the pharmacy previously they already have his info.
If he uses an inhaler, they know.

Apparently, if I start smoking, I can get it. Half tempted to walk into my CVS, grabbing a pack of Marlboros and slamming it on the pharmacy counter, and yelling, "Give me the damn shot!"
I think the reality of the situation is that anyone can get the vaccine this year if they want it and pay. Certain eligibility categories are not easy to confirm and your pharmacy is not going to investigate. Your insurance company may not pay for it so you may have to pay. That's the real change.
Some things are obvious: If the person has a prescription to treat a condition, the pharmacy can assume the person has that condition with near certainty. The age check requires an ID. But the health care worker and family member of a high-risk person exception / allowance is hard to verify.
All I was able to do was enter my DOB. CVS said I wasn't eligible at that point. There was no option to enter pre-existing conditions, high-risk factors, etc. Just denied with no explanation as to why. I'm going to go ask them why today.

lesser frequently met criteria. It might just be that we are in a period of flux now and the big retail companies have not yet had time to adopt to the new criteria which aren't official yet.
