I think he had a mild heart attack. That happened to my grandfather. He had "cankles" just like that and then died shortly after. He had fluid leaking out around his ankles.

Yes, I'm certainly no doctor. I just remember the stories of him when I was a child. He had a heart attack in his garden just after he retired from working as a metallurgist for Hughes Tools (Howard Hughes). He was only about 66 years old and did not eat well, smoked cigars. Lived a few days after.
