I think it does not necessarily spin but a wheel was usually used to transform the push and pull motion into a more linear motion (eg train moving forward) and I can’t think of an application right now where this was not the case

The application of the original steam engines by Newcomen and Watt was pumping, often a back & forth linear motion, and what I thought was happening here for about 5 minutes.
