Imagine being a healthcare provider sending people bills for $8.31. why?! Just put it on my tab

I got one for about $5.00 the other day. At what point is sending the bills out more costly than what you get back? Especially if I send back in a check and someone has to open it and process it.
It's not only that, it's the concept of spending something like $8-10k a year on premiums and then getting these annoying bills? Like instead of charging me $10,000 next year, charge me $10,008.17 like wtf is wrong you people?
Fair point.
