Instead, they are banning vaccines, spreading misinformation about vaccines, initiating policies that make it harder for families to choose to get vaccinated and cutting healthcare and public health funding significantly.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
This could be things such as massive education campaigns, offering vaccines free of cost to everyone, in-school vaccinations for all children, incentivizing families to be up to date on their vaccinations and more.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
If Florida decided that having the highest level of intervention was too much for vaccine mandates, then a proper functioning public health unit would instead have already implemented and rolled out other interventions on that ladder.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
4- Sick children means sick staff, and ultimately less learning and an unstable learning environment. 5- Allows for a more equitable and proportional approach.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
2- Less intrusive interventions tend to not work as effectively and are often insufficient to allow us to achieve herd immunity. 3- Vaccination is a very effective prevention tool and generally benefits outweigh risks.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
School vaccine mandates generally sit at the highest level, typically close to "restricting choice" and for good reasons: 1- The risk of harm to others is high if children are unvaccinated. This includes infecting vulnerable children & their families And the staff and administration at the school.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
The Nuffield ladder of interventions helps determine ethical justifications and proportionality of interventions, which have to be weighed against the severity of the issue at stake, what the public health goals are and individual liberty and intrusiveness of the policy at hand.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
Public health frameworks fundamentally require that the level of intervention be proportional to the risks being addressed. Let's talk about what public health really means in terms of policy and intervention.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
A while ago I talked about the Nuffield Ladder of Interventions, a core principle in public health, but in the context of mask mandates. Today I want to talk about it in the context of vaccine mandates, particularly with the news coming from Florida.
Sabina Vohra-Miller (@sabivm.bsky.social)
My biggest issue with Florida removing vaccine mandates for all vaccines in schools is that they have not instead chosen another tactic or intervention to replace these. Which means they have effectively decided that public health is not a mandate they consider worthy of undertaking.

Sabina Vohra-Miller (@sabivm.bsky.social)
I feel so much for Americans, with the US restrict access to COVID-19 vaccines (whatever happened to choice?!!). Many have asked about obtaining doses in Canada, but because we have a predominantly public pay system for the COVID-19 vaccines, it will be challenging.

I feel it is highly unlikely for us to have any Nuvaxovid for the upcoming 2025-2026 vaccination campaign. However, with Sanofi acquiring the rights, it makes me very hopeful for vaccine supply and distribution for fall of 2026.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
Thanks to a tip, I emailed their Canadian office & was told they are working to accelerate transition. However, no Nuvaxovid for 2025 fall – 2026 spring season. When asked if there is a possibility to get from US counterparts for Canadian supply this year, they have “no info on such an acquisition”.
Sabina Vohra-Miller (@sabivm.bsky.social)
Okay, Nuvaxovid Update for Canada. This is the final of the Canadian vaccine updates for this fall, and unfortunately this is not a good one (at least for this year). As far as I can see, no application was put through Health Canada for approval this year either.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
I emailed them and got a similar response. I also inquired whether there were any plans to acquire stock from their US counterparts for this fall 2025 campaign and they said they had no information on that. So seems unlikely.

On Novavax, this is what I know so far. In 2021 we signed a deal to produce domestic vaccines. The deal to purchase Novavax doses was contingent on them being produced domestically, with a deadline of end of year 2024. That didn’t happen, so the deal was cancelled. I don’t know what happens next.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
The last I had heard on Novavax is that the facility did not get approval by the deadline of end of year 2024 and so we cancelled our deal to purchase Novavax vaccines (which was contingent on them being produced locally).

I have a feeling that all the medical associations will still continue to strongly recommend the COVID-19 vaccine (as they already have so far). Then we wait & see how each state adopts the recos, whether the risk factor list will continue to be as inclusive or if they will tighten it…

This was a very thorough and robust study, perhaps one of the strongest types of studies that could be conducted in such a large scale, where even small increases of adverse events could be discerned. Shouldn’t this be exactly what RFK Jr would want to see and do?

This is not practicing evidence-based medicine, it is instead going to actively harm pregnant and post-partum parents and their babies.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
As with everything, it needs to be a risk vs benefit discussion. Makary at the FDA instead is focusing solely on the risks of SSRIs (which aren't even clear, & if they do exist, are very low), without taking into consideration the benefits of these meds & the risks of untreated mental illness.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
Many studies have shown clearly that pregnant people with depression who receive proper treatment have better outcomes for both themselves and their babies versus those with untreated depression.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
2- But most importantly, untreated or not properly managed depression and anxiety carry huge risks, both especially to the pregnant person but also for the newborn.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
But even then, the absolute risk was very low & the number needed to treat to harm, a term we call NNTH was high. NNTH indicates how many need to be exposed to a risk factor for one person to experience a harmful outcome. This tells us the overall safety of SSRIs & SNRIs in pregnancy is very high.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
But there are two important takeaways: 1- The studies that show a slight association with birth defects or neurodevelopmental concerns have many flaws, so it is difficult to determine if their results are even valid.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
Majority of the studies show absolutely no increase in any kind of teratogenicity_or birth defects or neurodevelopmental concerns with the use of SSRIs and SNRIs in pregnancy. However a small handful have shown a very slight increase in heart defects or other birth defects and ADHD or Autism.
Sabina Vohra-Miller (@sabivm.bsky.social) reply parent
And so with that, there are of course always going to be concerns about the validity of the data and results since studies may not be rigorous or well-designed and there is always the potential to be susceptible to confounders or other variables that impact outcomes. So what does the data show?
