My understanding is that Medicare Advantage plans are required to cover all vaccinations covered by Medicare Part B at no cost. But your friend should double check with their specific plan.
Medicare Part B is required by statute to cover 4 vaccinations (pneumonia, hep B, flu, and COVID-19), without cost sharing. This requirement is not linked to ACIP/CDC approval. Private insurance companies may link coverage to ACIP/CDC.

Some states (e.g., NY) don't allow vaccinations that have not been approved by CDC's Advisory Committee on Immunization Practices without a prescription, while others such as NJ do. ACIP was scheduled to meet on the new COVID vaccinations on 9/18 but that meeting has been delayed.
During the early pandemic years (when vaccinations were in short supply in many places), a lot of folks went out of state to get their COVID shots so I think you should be able to now. I know folks in VA who are already scheduling appointments in MD.
