That's the job of an executive, to have the ideas and give your engineers goals to achieve. You do need sufficient knowledge of the business to do this, but it's fundamentally not "engineering." Elon is an executive, and an increasingly poor one at that.
You don't have to have a degree, but you do need to work at designing, building and/or testing things. I don't think Elon is involved at that level, he's more of a high-level vision/idea guy, to the extent he's involved in day-to-day operations of his companiea.
