SARS-CoV-2 infection weakens immune-cell response to vaccination NIH-funded study suggests need to boost CD8+ T cell response after infection. www.nih.gov/news-events/...

Vaccination of people who had been infected with #SARSCoV2 prior to vaccination produced spike-specific CD8+ T cells at considerably lower levels and with less functionality.

The researchers found that vaccination of people who had never been infected with SARS-CoV-2 induced robust CD4+ and CD8+ T-cell responses to the virus’ spike protein. In addition, these T cells produced multiple types of cytokines. However, people who had been infected with SARS-CoV-2 prior to vaccination produced spike-specific CD8+ T cells at considerably lower levels and with less functionality. Moreover, the researchers observed substantially lower levels of spike-specific CD8+ T cells in unvaccinated people with Covid-19 than in vaccinated people who had never been infected.

Watch out for Mast Cell Activation Syndrome (MCAS) after SARSCov2 infections, especially if you have #LongCovid It can be deadly if you are unprepared for an anaphylaxis episode. www.aaaai.org/conditions-t...

MCAS is a condition in which the patient experiences repeated episodes of the symptoms of anaphylaxis – allergic symptoms such as hives, swelling, low blood pressure, difficulty breathing and severe diarrhea. High levels of mast cell mediators are released during those episodes.

MCAS is a condition in which the patient experiences repeated episodes of the symptoms of anaphylaxis – allergic symptoms such as hives, swelling, low blood pressure, difficulty breathing and severe diarrhea. High levels of mast cell mediators are released during those episodes. The episodes respond to treatment with inhibitors or blockers of mast cell mediators. The episodes are called ā€œidiopathicā€ which means that the mechanism is unknown - that is, not caused by allergic antibody or secondary to other known conditions that activate normal mast cells. Evaluation for MCAS starts with determining whether the symptoms occur in separate attacks and are typical symptoms of an anaphylactic reaction without a clear cause. Mast cell mediators increase during the episode. Those mediators should be measured during acute episodes and at baseline looking for elevations during symptoms. Finally, the improvement with treatment using inhibitors of mast cell mediators completes the diagnosis. Symptoms The symptoms most consistent with anaphylaxis are: • Heart related symptoms: rapid pulse (tachycardia), low blood pressure (hypotension) and passing out (syncope). • Skin related symptoms: itching (pruritus), hives (urticaria), swelling (angioedema) and skin turning red (flushing). • Lung related symptoms: wheezing, shortness of breath and harsh noise when breathing (stridor) that occurs with throat swelling. • Gastrointestinal tract symptoms: diarrhea, nausea with vomiting and crampy abdominal pain. Mediators Mast cells are known to produce many molecules that cause inflammation, but only a few mediators or their stable breakdown products (metabolites) have been found reliably elevated in episodes of MCAS and measurable in commercial laboratory tests. Increases in serum mast cell tryptase and in urine levels of N-methylhistamine, 11B -Prostaglandin F2α (11B-PGF2α) and/or Leukotriene E4 (LTE4) are the only useful tests in diagnosis of MCAS

Mast cells release histamine, tryptase, chymase, cytokines (TNF, IL-6), prostaglandins, etc., and are activated by stress, infections, tissue damage, antibodies, hormones, and other mediators.

Long COVID-19: a Four-Year prospective cohort study of risk factors, recovery, and quality of life Sanaa M. Kamal, Mohammed S. Al Qahtani, …Faisal S. F. AlHarbi Show authors BMC Infectious Diseases volume 25, Article number: 1082 (2025) Cite this article Abstract Purpose Long COVID-19 is a growing public health concern, but its long-term burden and predictors remain underexplored, particularly in underrepresented populations. Methods This four-year prospective cohort study was conducted in Saudi Arabia, enrolling adults with confirmed acute COVID-19 from multiple affiliated healthcare centers between March 2020 and March 2024. Of 1,521 screened patients, 816 were enrolled and followed for up to four years (median: 24 months). Per WHO criteria, participants were classified as having long COVID-19 (n = 238) or resolved infection (n = 578). Demographics, comorbidities, vaccination, reinfection, and acute illness severity were recorded. Health-related quality of life (HRQoL) was assessed using SF-36 and EQ-5D-5 L. Logistic regression identified predictors of long COVID-19, and Cox proportional hazards models evaluated time to recovery. Results Fatigue (57.1%), post-exertional malaise (45.8%), cough (41.2%), and cognitive dysfunction (30.7%) were the most common persistent symptoms. Female sex (adjusted OR 11.11; 95% CI: 4.48–26.24) and diabetes mellitus (adjusted OR 14.3; 95% CI: 7.0–29.4) independently predicted long COVID-19. Delayed recovery was associated with female sex (aHR 3.36; 95% CI: 1.85–6.10), diabetes (aHR 1.57; 95% CI: 1.00–2.46), reinfection (aHR 1.86; 95% CI: 1.05–3.29), and hospitalization (aHR 1.08; 95% CI: 1.01–1.16). HRQoL scores remained significantly lower at 6 and 12 months. In the long COVID-19 group, 38.7% of patients normally resumed work within 12 months, compared to 82.3% in the resolved COVID-19 group. Conclusions Nearly 29% of post-acute COVID-19 patients developed long COVID-19 in this Middle Eastern cohort. Female sex, diabetes, reinfection, and hospitalization predicted delayed recovery. Persistent symptoms and impaired HRQoL highlight the need for early risk stratification and structured post-COVID care

The Trump administration is more concerned about conducting raids on fire crews than protecting communities from catastrophic fires. Firefighters put their lives on the line, Wyden emphasized, including the Oregon firefighter who died on Sunday while battling a wildfire in south-western Montana.

The firefighter, whose name has not been made public, has lived in the US for 19 years after arriving with his family at the age of four. He received a U-visa certification from the US attorney's office in Oregon in 2017 and submitted his U-visa application with US Citizenship and Immigration Services the following year. The U-visa program was established by Congress to protect victims of serious crimes who assist federal investigators, and the man has been waiting since 2018 for the immigration agency to decide on his application, according to Stephen Manning, a lawyer with Innovation Law Lab, a Portland-based nonprofit representing the firefighter. Another homeland security policy says agents cannot detain people who are receiving or have applied for victim-based immigration benefits, his lawyer said. Charging the man with an immigration violation was
