Funny, I scheduled my COVID shot with CVS, and they didn't require an Rx. I just had to do the online checkboxes. This Rx requirement appears to be targeted only to blue states by the CDC and HHS (specifically, PA, which has been critical of Kennedy, and didn't elect Oz as their senator last year).
Doctors' notes are required for exemptions, but not to give the vaccine. I am not aware of any pharmacy chain that requires a doctor's verification to confirm a patient's eligibility for a vaccine.
