depends what you're cooking in, i test it by getting my hand wet and splashing small drops of water in the pan (very small amount!). if the drops slide around a bit before boiling away its hot enough. if they just splash and sizzle right away (or don't sizzle at all) then its not hot enough.
this could be an issue with the pan/cooking surface not being preheated. it starts off cooking slow then suddenly its way too hot and burns, meaning the bread was added when the pan was still heating up and it ends up being too hot once it catches up. just speaking from my experience.
