For those who are curious, this is what an end of month breakdown looks like for me patient appointments: 182 total patient revenue (oop and insurance): $2749.00 $/patient: $15 so for services patient services provided: 419 $/service: $6.65 per service (grants/donations were $61,000 this month)

Our story is usually in number of uninsured cared for, as well as birth outcomes (mother and child). It's difficult to track much more of a community health improvement without in house research and attempts to get funding with a UA component have been ... terrible.

I originally did per service but got shamed out of it because that's what crisis pregnancy centers do to make themselves look more impactful than they are. But also if you were a for profit clinic, you bill per service, so it really is more precise.

I ask because I track both and I usually break down our impact by number of patients divided by income coming directly from patients (out of pocket or reimbursement), but our patient number is starting to level out but the services per patient is exploding.

If you were looking at a nonprofit clinic's numbers, which do you think is more representative: number of individual patient appointments (patient may be seen more than once in a time period so not completely specific) or number of services provided, knowing patients often receive multiple per appt?

Exactly. And red state public health is utterly dependent on resources for capacity. We don’t “ban” things as much as underfund them so they can’t be accessible. A sister city project would help that exponentially.

On this glorious long weekend I’ve spent most of it trying to figure out whether we can be an access point for vaccines - either on site or with prescriptions. A thing we’ve already been working on for HPV (still a crisis) and birth control (so difficult to get). But we need resources to do it.

Stockpiling mife while red states had complete bans. Paying for travel while letting red clinics close, regardless of the other health care we could provide. Offering shield protections to blue providers while red recipients were left with most of the immediate legal risk.

Because as soon as abortion was “sent back to the states” the end result was a coalition of blue states protecting their own, with the resources primarily pouring into them and then being divvied out to red staters who tapped into those networks.

Not only highest costs, but UPFRONT costs. In our city, if you have private insurance you are expected to pay your deductible to the OB at 20 weeks or your care is cut off. We end up with a number of patients coming to us later in pregnancy because they can’t pay to continue.

So, not that it does any real world good, but I’d like to point out that Medicaid has at least $200k in reimbursements we haven’t received in the last 3 years because they won’t pay us for prenatal care because no local doctor will agree to deliver our patients. There is your real Medicaid fraud.

This woman defrauded Medicaid out of approximately $20,000 a year. She most likely made around $45,000 per year. She was clearly supporting her disabled child (likely an adult based in age). And she did this via $60 ride vouchers. Which would be about 160 round trips per year.

My mother was in a school shooting in 2011 - the student killed two teachers including one who was her good friend. She was never okay after. She took an early retirement buy out in 2013 and died two years after. I still blame much of her mental and physical decline on that event.

Did you know that if you are pregnant in Alabama and need a diagnostic ultrasound - say for an ectopic or to check bleeding - and your Medicaid hasn’t been approved yet, you have to pay out of pocket? So, yeah, there goes another $250 out of our patient support fund….
